permeation enhancer

A permeation enhancer is an inactive ingredient added to a formulation to make the skin or a mucous membrane temporarily more “leaky,” so a drug that would otherwise crawl across the barrier can pass through faster. Think of it as loosening the mortar between the skin's bricks just long enough for the drug to slip through, ideally without lasting damage.

Enhancers work by several mechanisms: some (like oleic acid, terpenes, or certain fatty acids) disrupt or fluidize the tightly packed lipids of the stratum corneum; some (like Azone or surfactants) interact with skin proteins or extract lipids; and solvents such as ethanol, propylene glycol, or DMSO increase how much drug dissolves in and partitions into the skin. In Fickian terms they raise either the drug's effective diffusion coefficient through the barrier or its partitioning into it, both of which increase flux.

The central tension is reversibility versus irritation: a good enhancer should boost permeability strongly but transiently, with the barrier recovering afterward and minimal redness or sensitization. Aggressive enhancers can cause irritation, and effects vary widely between individuals and body sites. The same concept applies beyond skin — enhancers are also used to improve absorption across nasal, buccal, and intestinal mucosa for drugs that otherwise permeate poorly.

Ethanol is one of the most widely used enhancers, but it can also extract or evaporate alongside the drug, sometimes changing the formulation's drug concentration over time — a stability point that must be designed around.
