Imagine the electrical wiring of a house. The brain and spinal cord are the central fuse box; everything beyond them, running out to the muscles and skin of the arms, legs, and trunk, is the peripheral wiring. Electrodiagnostic medicine is the trade of testing that wiring, so before any of the tests make sense you need a quick map of what the wires are and how they connect.

Signals heading out to muscle start at a motor nerve cell in the spinal cord and travel down a chain of structures: the nerve root, where the wire first leaves the spine between two vertebrae; the plexus, a tangle near the shoulder or pelvis where roots cross over and rebraid like cables in a junction box; the named peripheral nerve (median, ulnar, sciatic, and so on) that carries the signal the rest of the way; the neuromuscular junction, the tiny chemical gap where nerve hands its message to muscle; and finally the muscle fibre itself, which contracts. Sensation runs the same road in reverse, from skin back toward the cord. A break, squeeze, or disease can strike at any one of these stations.

This anatomy is the whole point of an electrodiagnostic study. The physiatrist is not just asking is the nerve damaged but where along the chain is the problem, because a numb thumb from a pinched wire at the wrist (carpal tunnel) is a completely different repair job from the same numbness caused by a pinched nerve root in the neck. Reading the body's wiring means knowing the wiring diagram first.

A patient says their whole hand has gone numb. The peripheral wiring map tells the examiner where to look: the median nerve covers the thumb side, the ulnar nerve the little-finger side, and the C8 nerve root and lower plexus feed both. Numbness that respects one nerve's territory points to a peripheral nerve; numbness in a band that crosses several nerves points further upstream toward a root or plexus.

The same complaint, numb hand, points to different stations depending on the pattern.

The central nervous system, the brain and spinal cord, is not what electrodiagnostic studies test; a normal nerve study does not rule out a problem in the cord or brain, which need imaging or other tests.
