Picture two ways a building can come down. In one, a demolition crew dismantles it piece by piece, carting the rubble away neatly. In the other, the building is hit by a wrecking ball, walls burst outward, and debris and dust scatter everywhere, prompting alarms and a cleanup crew to rush in. Apoptosis is the tidy first version; necrosis is the violent second one. Necrosis is cell death caused by severe, sudden injury, in which the cell swells and bursts rather than being neatly packaged.
Necrosis is classically described as accidental, unplanned death, although scientists now know some forms are actually regulated (see necroptosis). In the common, injury-driven kind, a damaging shock — lack of oxygen, extreme heat or cold, poison, physical trauma, or infection — overwhelms the cell. Its energy supply collapses, so the pumps that control water balance fail. The cell and its organelles swell with water until the plasma membrane ruptures. When it bursts, all the internal contents — enzymes, signaling molecules, cell debris — spill into the surrounding tissue. The immune system reads these spilled contents as a danger signal and triggers inflammation: redness, swelling, heat, and the recruitment of cleanup cells.
The key contrast with apoptosis is the mess and the aftermath. Apoptosis is quiet, controlled, energy-requiring, and provokes no inflammation; necrosis is sudden, leaky, and inflammatory. That inflammation is why necrosis underlies much of the tissue damage in heart attacks, strokes, frostbite, severe infections, and gangrene — the spilled contents can injure healthy neighbors and amplify the harm. Understanding which kind of death is occurring matters in medicine, because the two are managed very differently.
During a heart attack, a blocked artery cuts off oxygen to part of the heart muscle. Starved of energy, those cells can no longer run their water pumps; they swell and burst. Their contents spill out, triggering inflammation that can damage nearby tissue too. This is necrosis — the violent, leaky death that contrasts sharply with the tidy self-destruction of apoptosis.

Not all necrosis is purely accidental. Necroptosis is a genuinely programmed form that looks messy like necrosis but is switched on by specific proteins — so 'unplanned' is a useful first picture, not the whole story.
