Myopericarditis is inflammation that involves both the pericardium and the adjacent heart muscle (myocardium) at the same time. The two structures sit right next to each other, so an inflammatory process in one can readily spill over into the other; myopericarditis describes a picture dominated by pericarditis but with some muscle involvement as well.

It usually starts like ordinary pericarditis, with positional chest pain, a friction rub, and typical electrocardiogram changes. The clue that the muscle is also involved is a rise in cardiac troponin, a blood marker of heart-muscle injury. When muscle inflammation predominates instead, with significant troponin rise and reduced pumping function, the term myocarditis (or perimyocarditis) is used.

Most cases are triggered by viruses and tend to be self-limited, settling with rest and anti-inflammatory medicines, though vigorous exercise is usually avoided during recovery. The outlook is generally good when heart function is preserved, but doctors monitor pumping function and rhythm, because more extensive muscle involvement can occasionally weaken the heart or provoke arrhythmias.
