Take someone's relaxed arm and gently bend the elbow back and forth. Even when the person is doing nothing and trying to stay loose, you feel a faint, springy resistance — the limb is not floppy like a wet rope, nor stiff like a board. That quiet background resistance you feel in a muscle at rest is what clinicians call muscle tone. It is always there, keeping the body subtly ready, and you only really notice it when it is too low or too high.

Tone has two ingredients mixed together. One part is purely mechanical: muscles, tendons, and the soft tissue around joints have natural elasticity and stiffness, like a rubber band, even with no nerve signal at all. The other part is neural: a low-level loop in the spinal cord — the stretch reflex — keeps a trickle of activity in the muscle so it resists being lengthened. In a healthy person these are balanced and barely noticeable. The clinical test is simple: the examiner moves the relaxed joint and judges whether the resistance feels normal, reduced (hypotonia, a floppy limb), or increased (hypertonia, a stiff limb).

Tone matters in rehabilitation because so many neurological injuries change it, and the change shapes everything downstream. Too little tone makes a limb hard to position and use; too much makes movement, washing, and dressing difficult and can pull joints into fixed deformities over time. But the single most useful idea is that some tone is helpful — a person may lean on stiffness in the leg to stand. So the goal is rarely 'remove all tone'; it is to understand the tone, measure it, and decide whether and how to adjust it.

A therapist supports a patient's relaxed forearm and slowly flexes and extends the wrist. A normal wrist offers a soft, even resistance throughout. A hypotonic wrist flops with almost none. A hypertonic wrist pushes back, and the harder part of that push will lead to the questions: is it spasticity, rigidity, or something else?

Tone is the resistance felt when moving a relaxed joint — neither floppy nor board-stiff in health.

Tone is judged with the person relaxed and not helping the movement. If the patient voluntarily tenses or guards, you are no longer measuring tone — you are feeling effort, and the assessment is unreliable.
