Multiple sclerosis (MS) is a disease in which the body's own immune system strips the insulation off nerves in the brain and spinal cord, so signals travel poorly or not at all. Because the damage can land almost anywhere in the central nervous system, MS produces an unpredictable, shifting mixture of problems — weakness, stiff and spastic muscles, numbness, imbalance, tremor, bladder trouble, blurred vision, cognitive difficulty, and a heavy, characteristic fatigue. Rehabilitation in MS is the work of keeping people as functional, independent, and active as possible across a disease that may relapse, remit, or slowly progress for decades.

Because MS is so variable, rehabilitation is highly individualized and addresses whichever impairments dominate at a given time. Exercise — once wrongly thought harmful in MS — is now a cornerstone: it improves strength, fitness, walking, and mood without speeding the disease. Therapists manage spasticity, train balance and gait (often with walking aids or an ankle-foot orthosis for foot drop), conserve energy against the fatigue, and address bladder, swallowing, speech, and thinking. The team works alongside the neurologist's disease-modifying drugs, which slow the immune attack but do nothing for the disability already present.

A crucial honesty is the role of heat: many people with MS experience temporary worsening of symptoms when overheated (during exercise, fever, or hot weather), which alarms them but is not actual new damage and resolves on cooling — so cooling strategies let them exercise safely rather than avoid activity. Rehabilitation does not cure or stop MS; it manages the consequences, preserves function, and adapts to a moving target. Maintaining activity is protective, while the old advice to rest and avoid exertion did real harm.

An ankle-foot orthosis lifts the foot so she stops tripping; a graded exercise program done in a cool room, plus energy-conservation planning, lets her work part-time again — and a cooling vest keeps summer heat from stopping her in her tracks.
