metastasis and invasion

Imagine weeds in a single flowerbed. Annoying, but you can dig them out. Now imagine those weeds releasing seeds that drift across the whole garden and take root in distant beds. The garden is now far harder to clear. Cancer becomes most dangerous in the same way: when cells break free of the original tumor, travel through the body, and start growing new tumors somewhere else. That spreading is called metastasis, and the act of breaking into surrounding tissue is called invasion.
More precisely, invasion is when cancer cells push past their normal boundary — they loosen their grip on neighbors, dissolve the surrounding scaffolding (the extracellular matrix and basement membrane) with enzymes, and crawl into nearby tissue. Metastasis is the longer journey: invading cells enter blood or lymph vessels, survive the trip, exit at a distant site, and establish a new colony, called a secondary tumor. This is an inefficient, treacherous process for the cell — most wandering cancer cells die — but the few that succeed can seed tumors in the lungs, liver, bones, or brain.
Metastasis matters because it is responsible for the great majority of cancer deaths. A tumor confined to one spot can often be removed by surgery; once it has spread to multiple organs, it is much harder to treat. This is exactly why early detection is so emphasized: catching a cancer before it invades and spreads dramatically improves outcomes. A common misconception is that a metastasis in, say, the liver is 'liver cancer' — it is not; it is the original cancer (for example breast cancer) growing in a new location, and it is usually treated as such.
A breast tumor sheds a few cells into the bloodstream; most die, but one survives the journey, lodges in the bone, and grows. The new growth in the bone is still breast cancer — 'breast cancer that has metastasized to bone' — and is treated with breast-cancer therapies, not bone-cancer therapies.
A metastasis is the original cancer growing in a new place.
A liver metastasis from breast cancer is still breast cancer, not liver cancer; metastasis, not the original tumor, causes most cancer deaths.
