ionization

Ionization is whether and how a molecule carries electrical charge in solution, and it shifts with the surrounding acidity. The same drug can travel through the body as a neutral, charged-positive, or charged-negative species, like a chameleon changing color depending on the local pH.

An acidic group (such as a carboxylic acid) loses a proton and turns negative as pH rises; a basic group (such as an amine) gains a proton and turns positive as pH falls. Which form dominates is set by comparing the local pH to the group's pKa. Many drugs are zwitterionic or multiply ionizable, carrying several such groups at once.

Ionization state is a master switch for behavior. Charged forms are far more water-soluble but cross fatty membranes poorly, so a drug often needs to be ionized to dissolve in the gut yet neutral enough to be absorbed. It also dictates whether a molecule can form ionic interactions or salt bridges with its target.
