A manual wheelchair is the familiar chair on wheels that a person moves by pushing the large rear wheels with their hands (or that someone pushes for them). It is the most common form of wheeled mobility, and for many users it is not a sign of confinement but a tool of freedom — a way to go to work, travel, and live independently when walking is not possible.

Manual chairs come in two broad families. A folding chair has a cross-brace frame that collapses sideways for transport, which is convenient but adds weight and flex. A rigid chair has a fixed frame and usually quick-release wheels; it is stiffer, lighter, and pushes more efficiently because less of each stroke is lost to a flexing frame. Within these, weight matters enormously: standard chairs are heavy and basic, while ultralight chairs (made from aluminium, titanium, or carbon) are configured precisely to the user — the rear axle position, seat height, and backrest are all adjusted so the chair is easy to push and balance. A self-propelling user reaches the hand rims on the rear wheels and drives the chair; a user who cannot do this is pushed by an attendant.

The choice of manual chair has long-term health consequences, not just convenience. People who self-propel use their arms for the work the legs once did, day after day, for years. A heavy or poorly set-up chair forces harder, more frequent pushes and is a major cause of shoulder and wrist injury over a lifetime of use. This is why a good prescription leans toward the lightest, best-fitted chair the person can use and afford — protecting the shoulders is part of the prescription, not an afterthought.
