liquid biopsy

A traditional biopsy means cutting out a piece of a tumour with a needle or a scalpel — painful, risky, and impossible to repeat often. But a tumour is constantly shedding bits of itself into the bloodstream, including fragments of its own DNA. A liquid biopsy is the idea that you can learn about a cancer by simply drawing a tube of blood and reading those fragments — a biopsy from a liquid, not a solid.

Here is how it works. When cells die, they spill their broken-up DNA into the blood; this is cell-free DNA, and a small fraction of it in a cancer patient comes specifically from the tumour (circulating tumour DNA, or ctDNA). The trick is that the tumour's DNA carries the tumour's mutations, so a very sensitive test — usually digital PCR or next-generation sequencing — can sift these rare cancer fragments out of the ocean of normal DNA and read which mutations are present. The same approach lets prenatal testing read fragments of fetal DNA floating in a pregnant person's blood, screening for conditions like Down syndrome without a needle into the womb.

The promise is large: a blood draw can be repeated often, so doctors can watch in near-real-time whether a cancer is responding to treatment, catch resistance mutations as they emerge, or detect a recurrence before a scan would. The honest limits keep it from being a magic early-detection test for everyone: early or small tumours shed very little DNA, so a negative result does not rule out cancer; healthy older people accumulate harmless blood-cell mutations that can masquerade as cancer signals; and using these tests to screen healthy populations risks false alarms and overdiagnosis. It is a powerful monitoring tool that is still proving itself for screening.

Non-invasive prenatal testing is a liquid biopsy of pregnancy: a tube of the mother's blood contains fragments of the fetus's DNA, and counting how many fragments come from chromosome 21 can flag an extra copy (Down syndrome) — though a positive still needs confirmation by a definitive test.

A negative liquid biopsy does not rule out cancer — small early tumours shed little DNA — and harmless age-related blood-cell mutations can mimic a cancer signal, so it is not yet a proven universal early-detection screen.
