Lateral habenula as an antidepressant target
The lateral habenula is a small epithalamic nucleus that encodes negative outcomes and aversion and is hyperactive in preclinical models of depression. In rodents, burst firing of habenular neurons drives anhedonia and behavioral despair, and suppressing that bursting — including via the mechanism thought to underlie ketamine's rapid antidepressant action — reverses depression-like behavior.
This mechanistic story has made the lateral habenula an attractive stimulation target, and isolated human case reports of habenular DBS for severe depression exist. Human evidence is nonetheless very limited, and the nucleus is small and surgically challenging to reach and stimulate selectively. The lateral habenula exemplifies a target with a strong circuit rationale but a speculative clinical status, where extrapolation from rodent mechanism to human therapy is not yet established.
Much of the habenula-depression evidence is rodent; direct human therapeutic data remain anecdotal, so this target should be described as promising and mechanistically motivated rather than clinically proven.
