The international normalized ratio is a standardized number that tells how long blood takes to clot compared with normal. Think of it as a thinness dial: a higher INR means the blood clots more slowly, which is exactly the effect doctors aim for in patients taking the blood thinner warfarin.
It is derived from the prothrombin time, a clotting test, then mathematically adjusted so results are comparable between different laboratories and reagents around the world. This standardization is what makes the INR reliable for guiding therapy no matter where the test is run.
In warfarin therapy, the dose is tuned to keep the INR within a target range — high enough to prevent dangerous clots in conditions like atrial fibrillation or after a mechanical heart valve, but not so high that bleeding becomes a serious risk. Because diet, illness, and other medications shift the INR, patients on warfarin need regular monitoring; the newer direct oral anticoagulants do not require this routine testing.
