GnRH analog
A GnRH analog is a drug built to resemble gonadotropin-releasing hormone, the brain signal that ultimately drives the production of sex hormones. By imitating or blocking that signal, these agents act as a master switch on the entire reproductive axis, dialing the body's output of estrogen or testosterone up or, more often therapeutically, down.
The clever paradox lies in the agonists. The pituitary only releases LH and FSH in response to pulses of GnRH; flood it with a continuous agonist and, after a brief initial surge, the pituitary's receptors become desensitized and shut off, collapsing sex hormone production to near-castrate levels. Antagonists, by contrast, block the receptor directly and suppress the axis immediately, without the early flare.
This pharmacological off-switch is used wherever lowering sex hormones helps: shrinking hormone-driven prostate cancer, taming endometriosis and uterine fibroids, halting precocious puberty in young children, and as part of in-vitro fertilization protocols to control ovulation timing. The same continuous-versus-pulsatile principle works in reverse for diagnosis and for treating infertility, where a pump delivering GnRH in natural pulses can actually restore a dormant axis.
The initial testosterone 'flare' from a GnRH agonist can briefly worsen prostate cancer symptoms, so it is often covered for the first weeks with an androgen-receptor blocker — a problem antagonists avoid by acting immediately.
