empyema

The lung sits inside a thin, normally near-empty space called the pleural space, with just a film of lubricating fluid letting it glide as you breathe. In empyema, that space fills with infected pus — imagine a layer of thick, infected fluid trapped between the lung and the chest wall, pressing on the lung and refusing to clear on its own.

Empyema is a collection of pus in the pleural space, most often arising as a complication of pneumonia when infection spreads from the adjacent lung. The fluid becomes thick, cloudy and rich in bacteria and inflammatory cells, and over days it can organize into pockets divided by fibrous strands, making it harder to drain. The patient typically has ongoing fever, chest pain and breathlessness despite antibiotics.

Antibiotics alone are usually not enough because they penetrate thick pus poorly; the cornerstone of treatment is draining the infected fluid, classically through a chest tube, sometimes aided by clot-busting medication instilled into the space or by a surgical procedure to clean out organized pus. Diagnosis rests on sampling the fluid by thoracentesis and analyzing it.

Not every fluid collection beside pneumonia is empyema; a simple sympathetic effusion can settle with antibiotics alone, so analysis of the fluid distinguishes the two and decides whether drainage is needed.
