central sensitization

Turn a stereo's volume up too high and even a quiet song blares; the input is normal but the system is amplifying everything. Central sensitization is when the nervous system does the same thing with pain: the spinal cord and brain become turned up so high that ordinary signals, and even gentle touch, are felt as painful. It is one of the central ideas in modern chronic pain.

After an injury, or sometimes for reasons that are not fully clear, the pain-processing neurons in the spinal cord and brain can become more excitable and stay that way. The result is that pain is amplified out of proportion to any actual damage. Two telltale features appear. In hyperalgesia, something that should hurt a little hurts a lot. In allodynia, something that should not hurt at all, such as a light touch, the brush of clothing, or a warm shower, becomes painful. The pain often spreads beyond the original site, lasts long after tissues have healed, and is accompanied by fatigue, poor sleep, and difficulty concentrating, because the whole nervous system is in an over-revved state.

Recognising central sensitization changes everything in treatment. Continuing to look for and treat a damaged structure that has long since healed is futile and can make things worse; injections and surgery aimed at the original site often disappoint. Instead, the work is to turn the system's volume back down: education that the pain is real but not a sign of ongoing damage, graded movement and exercise, sleep and mood support, and medicines that act on the nervous system rather than at the tissue. This is the biology behind conditions such as fibromyalgia and much persistent back pain.

A woman recovered from a wrist fracture months ago, yet now the lightest brush of a sleeve over the skin makes her wince (allodynia), and a firm handshake is agony (hyperalgesia). The bone healed perfectly; her nervous system simply stayed turned up too high.

Allodynia (pain from gentle touch) and hyperalgesia (exaggerated pain) are the fingerprints of a turned-up nervous system.

Central sensitization is real, measurable biology, not weakness or imagination. But more scans, injections, and surgery aimed at the original site usually do not help, because the problem now lives in the nervous system, not the tissue.
