A chest radiograph is a single, quick X-ray picture of the chest — a shadow image made by passing radiation through the body and recording how much each tissue absorbs. Bone and dense structures look white, air-filled lungs look black, and the heart appears as a grey silhouette in the middle. It is one of the oldest and most widely available heart tests, often the first image a doctor sees.

For the heart, the chest film answers a few broad questions. The size and shape of the cardiac silhouette suggest whether chambers are enlarged; a heart wider than roughly half the chest on a standard front-to-back upright film hints at enlargement. The lung fields reveal congestion — when the left heart fails to clear blood, fluid backs up into the lungs and shows as hazy markings, redistributed vessels, or frank pulmonary edema. The film also shows the great vessels, fluid around the heart or lungs, and the position of pacemaker leads or prosthetic valves.

A chest radiograph is a screening and overview tool, not a precise measure of cardiac structure. It cannot directly assess valve function, ejection fraction, or coronary arteries, and a normal-looking heart on film does not exclude serious disease. Heart size on a front-to-back portable film, common in sick patients, is exaggerated and must be read with caution. For detail, doctors turn to echocardiography or cross-sectional imaging.

The ratio of heart width to chest width (cardiothoracic ratio) above about 0.5 on a proper upright film suggests cardiomegaly — but only on a standard front-to-back (PA) film, not on portable bedside images.
