Most people know everyday tiredness — the kind a good night's sleep fixes. Cancer-related fatigue is something different and far worse: an overwhelming, persistent exhaustion that is out of proportion to any activity and is not relieved by rest. People describe legs like lead, a foggy mind, and a heaviness that swallows whole days. It is the single most common and most distressing symptom reported by people with cancer, during treatment and often for months or years afterward.

It is not simply being out of shape, though deconditioning makes it worse. Many factors feed it at once: the cancer itself, chemotherapy and radiation, anemia, poor nutrition, pain, disturbed sleep, depression and anxiety, and certain medicines. The rehabilitation approach starts by treating the reversible contributors — correcting anemia or thyroid problems, easing pain, improving sleep and mood — and then, crucially, by prescribing graded exercise. Energy conservation and pacing help people spend their limited reserve on what matters, and education reassures them that the fatigue is real, expected, and manageable rather than a sign the cancer is winning.

The most important and counterintuitive truth is that the instinctive remedy — rest — usually makes cancer-related fatigue worse, because inactivity deepens deconditioning and feeds the cycle. Across many studies, supervised, individualized exercise is the most consistently effective treatment. That said, fatigue must be respected: exercise is graded and balanced with rest, and any sudden severe fatigue is investigated for a treatable cause rather than simply pushed through.

A man midway through chemotherapy can no longer climb a single flight without resting and naps most of the day. His team checks for anemia, sets a short daily walk that grows week by week, and teaches pacing — within a month he is napping less and walking the dog again.

For cancer-related fatigue, graded exercise — not extra rest — is the most consistently effective treatment.

The natural instinct to rest usually worsens cancer-related fatigue. Graded exercise is the best-supported treatment, but a sudden change in fatigue still warrants a search for a treatable cause.
