basal secretion

Basal secretion is the low background rate at which a hormone is released when nothing special is going on — no meal, no stress, no stimulus calling for extra. It is the idle of an engine: the minimum output that keeps the system ready and ticking over, on top of which stimulated bursts are added when needed.

Insulin is a good illustration. Between meals and overnight the beta cells release a steady low trickle of insulin, the basal secretion, which restrains the liver from pouring out too much glucose; on top of this comes the much larger surge that follows a meal. This distinction is built directly into modern insulin therapy, which separates a long-acting basal insulin from short-acting mealtime doses.

Measuring a hormone in the basal state — a fasting, resting, unstimulated sample — is the starting point of much endocrine testing, because it gives a clean baseline against which a stimulation or suppression test can be compared. A basal level alone, however, can be ambiguous, since the same number may be normal at rest but inappropriate if it fails to change when challenged.
