antipyretic

An antipyretic is a drug that brings down a fever. Fever is not a malfunction but a deliberate setting: when the body fights infection, it raises its internal thermostat to a higher target, then shivers and conserves heat to reach it. An antipyretic works by resetting that thermostat back toward normal, not by cooling the body from outside like a cold flannel.

The thermostat lives in a brain region called the hypothalamus, and it is pushed up by a particular prostaglandin (PGE2) made there in response to inflammatory signals from the infection. Most antipyretics — paracetamol, aspirin, and other NSAIDs — lower fever by blocking the cyclooxygenase enzyme that produces this prostaglandin, so the set-point drops and the body lets its temperature fall. Notably they do not lower a normal temperature, because they only undo the upward push.

It is worth remembering that fever itself is usually a helpful defence, and lowering it mainly improves comfort rather than curing the illness. Antipyretics ease the aches, chills, and misery of a fever and can prevent dangerous overheating, but they do not shorten most infections. The honest reason to use them is to feel better — and in children, to avoid distress — not to "break" the fever as if the number itself were the disease.

Aspirin is avoided as an antipyretic in children with viral illness because of its link to Reye's syndrome, a rare but devastating brain and liver injury; paracetamol or ibuprofen is preferred.
