antiepileptic
An antiepileptic is a drug that prevents or reduces seizures — the bursts of abnormal, synchronized electrical firing that sweep through groups of brain neurons. The general aim is to make neurons harder to fire in runaway, repetitive bursts without shutting normal brain activity down.
Antiepileptics achieve this through several mechanisms. Some block voltage-gated sodium channels to limit rapid repetitive firing (phenytoin, carbamazepine, lamotrigine); some enhance GABA inhibition (valproate, benzodiazepines, phenobarbital); some block certain calcium channels (ethosuximide for absence seizures); and some act on the synaptic vesicle protein SV2A (levetiracetam). Many newer agents combine actions.
Beyond epilepsy, these drugs are widely used off the original label: as mood stabilizers in bipolar disorder, for neuropathic pain, and for migraine prevention, reflecting their broad influence on neuronal excitability.
Treatment usually starts with a single drug titrated slowly to balance seizure control against side effects like drowsiness, dizziness, and cognitive slowing. Many interact through liver enzyme induction or inhibition, several pose risks in pregnancy (especially valproate), and stopping them abruptly can provoke rebound seizures.
Carbamazepine, which blocks sodium channels, is used both to control focal seizures and, separately, to relieve the stabbing pain of trigeminal neuralgia.
The same membrane-stabilizing action serves seizures and neuropathic pain alike.
Several antiepileptics double as mood stabilizers and as neuropathic-pain agents, a good reminder that a drug's 'class' is named for its first use, not the limit of what it can do.
