angiotensinogen

Angiotensinogen is the raw material from which the body’s blood-pressure hormones are carved. By itself it does nothing to blood pressure; it is the silent starting block waiting at the top of the cascade for the signal to begin.

It is a protein made and steadily released by the liver, so it circulates in the blood at all times in large amounts. The pressure-raising cascade begins when the kidney’s enzyme renin clips angiotensinogen to release angiotensin I. Angiotensin-converting enzyme then trims angiotensin I into active angiotensin II. Because angiotensinogen is plentiful, the speed of the whole system is normally set not by how much substrate is present but by how much renin the kidney releases.

Even so, the amount of angiotensinogen can matter. Estrogen raises its level, part of why some women see blood-pressure changes on the pill or in pregnancy, and inherited differences in the angiotensinogen gene have been linked to blood-pressure variation. Newer drugs that lower angiotensinogen production with RNA-based therapy are being studied as a long-acting way to treat high blood pressure.
