Deep in the lung, beyond where mucus and cilia reach, the alveolar macrophage is the cleanup crew. Picture a roaming security guard patrolling the millions of tiny air sacs, swallowing any dust speck, bacterium, or dead cell it finds. It is the lung's resident eat-it-all defender.

Macrophage means roughly large eater, and that is exactly what these cells do: they engulf particles and microbes by a process called phagocytosis, then break them down internally with enzymes. They sit on the surface of the alveoli, where the thin gas-exchange membrane cannot afford a sticky mucus layer, so a mobile scavenging cell takes over the job of keeping the surface clean.

Beyond simply eating debris, alveolar macrophages help launch and calm immune responses by releasing signaling molecules, and they recycle worn-out surfactant. Their work is mostly silent, but they have limits: heavy dust loads, as in coal or silica exposure, can overwhelm and kill them, and some pathogens — notably the tuberculosis bacterium — survive by hiding and multiplying inside the very macrophage meant to destroy them.
